I don't think JKR would do it either--it's a great subject for Dark!Fic though. Although she might do it to make some point about punishing bad beliefs--in this universe people can go mad from magic, remember. So it wouldn't have to be something that happened naturally to still be "what he deserved" in a lot of people's eyes. And then there's also the possibility somebody can *look* mad when they're not because they know things. Harry is sometimes considered mad when he knows things, and part of what drives him crazy (pun intended) about Draco is that Draco is the one person who sometimes knows he's telling the truth but is still encouraging people to think he's crazy.
I don't think emotion alone can make you mad either...I mean, it can definitely make you mad in the moment. But fevered, to me, implies a sickness in the mind--fevers bring delusiouns, fever dreams. It's not a fever like a passion it's having your perceptions heightened/changed. And I do wonder if JKR is going for something like that when Draco is looking at something horrible and seeing it as something else--sort of like Kay looking at his pieces of ice. Usually Draco is much more reliable in his reactions--it's rare that he's not scared by something scary.
